About The Position

Heartline Staffing is hiring a Center Clinical Medical Director to lead a primary care center in Jacksonville, Florida, for a physician-led, mission-driven organization focused on high-quality, value-based care for seniors. The position combines clinical care for a small panel of seniors—generally living with three to five chronic conditions—with leadership and development of the center’s primary care physicians and nurse practitioners. The care model emphasizes personalized, respectful, accessible care and improved health outcomes and patient experience.

The role includes clinical leadership, support for center operations and financial performance, and contributions to center growth. The practice offers in-house consulting specialists, holistic health services, and door-to-door patient transportation. Local and occasional regional travel is required. A structured training program covers primary care, medical director and center operations, leadership pathways, medical economics, financial performance, sales, and growth.

Requirements

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
• Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
• At least one year of clinical experience in a geriatric or family practice setting.
• Ability to travel locally and occasionally regionally.
• Strong leadership, communication, and analytical skills.
• Proficiency in Microsoft Office.

Responsibilities

• Provide clinical leadership for the center.
• Manage and develop the center’s primary care physicians and nurse practitioners.
• Deliver value-based primary care to seniors with multiple chronic conditions.
• Care for a small patient panel of up to 60–90 patients.
• Support center operations, medical economics, and center financial performance.
• Contribute to center growth.
• Travel locally and occasionally regionally.
