Performs comprehensive assessments for persons seeking long term care services and needing public funding or home and community-based services. This role involves face-to-face visits with consumers to complete thorough assessments, reviewing medical and healthcare needs to issue the most appropriate and least restrictive Level of Care. The position requires working with family members, significant others, physicians, hospitals, and/or agencies to gather necessary information. Assessments must be completed within a standard of 10-business days. The role involves using Tablets and knowledge of WellSky and PIA software to enter assessments according to Pennsylvania Department of Aging (PDA) guidelines. Additionally, the Caseworker completes required forms for Dom Care Home and Personal Care Home residents applying for state SSI supplement, performs Level II assessments, and maintains accurate records of consumer assessments and other information for Agency and PDA reporting, adhering to HIPAA requirements. The position also involves preparing for appeal hearings and providing testimony. Caseworkers are required to use their own transportation for home visits and other job-related appointments.
