About The Position

This role is with the Community Transition Waiver team, focusing on providing case management to individuals transitioning from Intermediate Care Facilities into Home and Community-Based Services. The specialist will educate individuals and families about available services and options, inform them about their rights and Person Centered Planning goals, and complete field work for support plans, budgets, and assessments. The position also involves coordinating and referring services with various community partners and collaborating with other agencies to promote program goals.

Requirements

  • Has a Bachelor's Degree with at least 1 year of relevant experience working directly with people with disabilities.
  • Has knowledge of Home and Community Based Services (HCBS) and Settings Rule.
  • Has obtained certification as a QIDP (Qualified Intellectual Disability Professional).

Responsibilities

  • Provide case management to people who transition from Intermediate Care Facilities into Home and Community-Based Services.
  • Provide education to individuals and families regarding Home and Community-Based Services and available service options.
  • Provide information concerning rights, options, benefits, services, Person Centered Planning goals and expectations.
  • Complete field work assignments used to implement Person Centered Support Plans (PCSP), budgets, RFS tasks, Charting the Life Course, and UCANS assessments.
  • Coordinate and refer services of community resources, placement providers, collateral agencies, the courts, and/or other community partners.
  • Collaborate with other state and local programs, organizations and agencies, and with various stakeholders to promote program goals and objectives.
  • Provide consultation, make recommendations, give appropriate advice, and/or assist to facilitate decisions.
