About The Position

This position is with the Community Transition Waiver team. The role involves providing case management to individuals transitioning from Intermediate Care Facilities into Home and Community-Based Services. The Caseworker Specialist II will educate individuals and families about Home and Community-Based Services and available options, including information on rights, options, benefits, services, and Person Centered Planning goals. Responsibilities include completing field work for Person Centered Support Plans (PCSP), budgets, RFS tasks, Charting the Life Course, and UCANS assessments. The role also involves coordinating and referring services with community resources, placement providers, collateral agencies, the courts, and other community partners. Collaboration with other state and local programs, organizations, and agencies is key to promoting program goals. The position also provides consultation, recommendations, advice, and assistance in decision-making.

Requirements

  • Has a Bachelor's Degree with at least 1 year of relevant experience working directly with people with disabilities.
  • Has knowledge of Home and Community Based Services (HCBS) and Settings Rule.
  • Has obtained certification as a QIDP (Qualified Intellectual Disability Professional).

Responsibilities

  • Provide case management to people who transition from Intermediate Care Facilities into Home and Community-Based Services.
  • Provide education to individuals and families regarding Home and Community-Based Services and available service options.
  • Provide information concerning rights, options, benefits, services, Person Centered Planning goals and expectations.
  • Complete field work assignments used to implement Person Centered Support Plans (PCSP), budgets, RFS tasks, Charting the Life Course, and UCANS assessments.
  • Coordinate and refer services of community resources, placement providers, collateral agencies, the courts, and/or other community partners.
  • Collaborate with other state and local programs, organizations and agencies, and with various stakeholders to promote program goals and objectives.
  • Provide consultation, make recommendations, give appropriate advice, and/or assist to facilitate decisions.
