About The Position

This position involves maintaining an assigned caseload of weekly billable insurance hours and providing direct community-based behavioral health services and support to individuals and families. The role requires participation in the person-centered planning process, monitoring consumer progress, and ensuring timely completion of tasks. Confidentiality, professional conduct, and adherence to ethical standards are paramount. The Case Worker will also collaborate with a multidisciplinary team, engage in continuing education, and maintain accurate service notations according to agency and Medicaid standards. Flexibility is needed to respond to client needs and crisis situations, and services may be delivered in various community environments.

Requirements

  • One-year documented experience in the delivery of mental health treatment to children and /or adolescents.
  • High School diploma
  • Human service field with less than one year of full-time, post-graduate degree accumulated MH/DD/SA experience with the population served, or a substance abuse professional with less than one year of full-time, post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling.
  • Must be highly organized and detail-oriented with ability to be self-directed and motivated, and to securely manage confidential information.
  • Inquisitive and creative problem-solver; ability to design and maintain systems around staff members’ needs as much operational requirements.
  • Excellent written and verbal communication skills; able to communicate effectively and respectfully with consumers with varying beliefs, behaviors, orientations, identities, and cultural backgrounds.
  • Proficient in Microsoft 365 products, SharePoint, Google Docs.

Nice To Haves

  • Additional 5 years’ mental health experience preferred (with High School diploma).
  • Supervision shall be provided by a qualified professional with the population served until the individual meets one year of experience.
  • Experience across multiple languages and cultures, particularly with refugees or immigrant populations with diverse cultural and socioeconomic backgrounds.
  • Experience in information technology, data analytics, grant writing and development of new behavioral health services.
  • Preferred experience using Paylocity, Vairkko, and/or Ontarget.
  • Exhibits friendly, calm, and professional demeanor for in-person and virtual interactions with staff, consumers, stakeholders, partners, etc.

Responsibilities

  • Maintain assigned caseload of weekly billable insurance hours.
  • Provide direct community-based behavioral health services and support for the individual and family as indicated in the Person-Centered Plan.
  • Participate in the person-centered planning process.
  • Monitor and track consumer hours and annual intake dates and complete assigned tasks.
  • Complete OnTarget tasks assigned within 24 hours.
  • Assist the Case Manager in monitoring and evaluating the effectiveness of interventions.
  • Assist the Case Manager with consulting identified non-medical providers and engaging community/natural supports.
  • Ensure all information obtained is kept confidential.
  • Report critical concerns, problems, and lack of engagement to supervisor in a timely manner.
  • Participate in multi-disciplinary staffing and meetings.
  • Attend all appropriate in-services, workshops, or continuing education programs.
  • Complete appropriate annual intakes.
  • Maintain strict confidentiality according to all Federal and State guidelines and requirements.
  • Exemplify personal and professional conduct and uphold the Professional Code of Ethics.
  • Maintain a cooperative relationship with employees, treatment staff, and others.
  • Adhere to and support facility policies, programs, and activities.
  • Demonstrate tact, courtesy, and a positive approach to communication.
  • Responsible for personal development to ensure current knowledge in the profession.
  • Maintain a safe, clean, and drug-free environment in the office space utilized.
  • Willingness to work beyond what is required periodically.
  • Maintain service notation that meets agency notation standards, Medicaid standards, and/or any other auditing agency.
  • Deliver services in various environments, such as homes, schools, jails, homeless shelters, etc.
  • Perform any other job-related duties as specified by a supervisor.
