This position involves maintaining an assigned caseload of weekly billable insurance hours and providing direct community-based behavioral health services and support to individuals and families. The role requires participation in the person-centered planning process, monitoring consumer progress, and ensuring timely completion of tasks. Confidentiality, professional conduct, and adherence to ethical standards are paramount. The Case Worker will also collaborate with a multidisciplinary team, engage in continuing education, and maintain accurate service notations according to agency and Medicaid standards.
