This position is responsible for monitoring an assigned caseload of clients participating in a short-term life management program, establishing program plans and goals, and evaluating client progress through routine meetings. The Case Worker serves as an advocate for clients to acquire necessary services for functional coping. Additionally, the role assists with food pantry services by obtaining, receiving, organizing, and distributing supplies, assessing client needs, and maintaining records. Key responsibilities include interviewing applicants to determine eligibility for financial assistance and program enrollment, conducting needs assessments, establishing program goals, and referring ineligible applicants to other agencies. The Case Worker develops intake and case plans, instructs clients on completing paperwork, conducts orientations, and completes social histories. They are responsible for preparing and maintaining accurate case records and logs, entering data into the Homeless Management Information System (HMIS), and meeting regularly with clients to discuss progress. The role also involves enforcing disciplinary actions for house guideline infractions and managing food pantry operations, including inventory control, stock management, receiving deliveries, and monitoring product quality and expiration dates.
