This position is responsible for assessing, planning, coordinating, and evaluating the care of patients. The role involves conducting comprehensive sensitive assessments, developing care plans, arranging and coordinating service referrals, and providing ongoing monitoring. The Case Worker will refer clients/families to public benefits programs as eligible, maintain regular contact with enrolled clients, and document all activities in client records. Additionally, the position requires educating clients/families about the medical and psychosocial aspects of disease to optimize adherence to care, providing proactive follow-up, tracking symptoms and treatment responses, and adhering to all applicable state laws and department rules and regulations. The role also involves staying abreast of new resources, programs, and developments, and participating in continuous training and education.
