About The Position

As a Psychiatric Social Worker (PSW), the incumbent will coordinate the development, implementation, and monitoring of the patient's plan of care throughout the continuum by collaborating with the Attending Physician, the treatment team, and the patient/patient's representative. In addition, the PSW identifies and arranges for discharge planning needs in a timely manner. The Psychiatric Social Worker (PSW) is responsible for the initial and ongoing assessment of assigned patients to ensure that services provided are medically necessary and provided in the most appropriate setting.

Requirements

• Bachelor's in Nursing, -OR- Masters in Social Work or related field.
• 1-2 years discharge planning and/or utilization review experience in a health care or managed care environment.
• Licensed to practice nursing in the state of Florida -Masters in Social work (or related field) accepted in lieu of RN licensure.
• Excellent oral and written communication skills.
• Knowledge of community agencies and resources.
• Effective organizational skills.
• Computer experience (i.e., Word, Excel, Email).

Nice To Haves

• BSN preferred
• Certification in Case Management (CCM) preferred
• MSW preferred
• 1-2 years' experience in a psychiatric hospital setting.
