About The Position

Metropolitan Family Services (MFS) is seeking a full-time Case Manager for its South Chicago office. MFS is an organization that empowers families to learn, earn, heal, and thrive, acting as a change agent since 1857. The role focuses on making a positive impact on families and communities.

Requirements

  • Bachelor's Degree in Psychology, Social Work, or related field from an accredited college or university.
  • Ability to work evening and weekend hours as needed.
  • Satisfactorily pass all relevant screening and criminal background checks.
  • Driving for work required.
  • While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle, and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch.
  • The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing, and viewing a computer terminal.
  • Must be able to lift at least 30 lbs.

Nice To Haves

  • One year of case management experience in a social service or healthcare setting.
  • Certified Alcohol and Drug Counselor certification.
  • Bilingual in Spanish.
  • Crisis stabilization experience.
  • Transportation of clients preferred in a personally owned vehicle.

Responsibilities

  • Conducts thorough assessment of clients, evaluating problem and service needs.
  • Develops a written client service plan specifying goals and services.
  • Supplements service plans to meet client needs.
  • Provides supportive help, advocacy, and case management to individuals and families in coping with environmental and interpersonal relationship issues.
  • Works collaboratively with community resources, developing knowledge of resources and helping clients utilize them.
  • Works within contract, grant, and program requirements, including meeting timelines and reporting requirements.
  • Engages clients in challenging settings to learn/practice/develop healthy behaviors and coping skills.
  • Reassesses and monitors client situations and the adequacy of service plans.
  • Collaborates and participates with other program staff in planning and implementing service plans.
  • Handles emergency and crisis situations within the target population after consultation with a supervisor.
  • Authorizes limited amounts of financial assistance to own clients with supervisory approval.
  • May work with community groups.
  • Works effectively with diverse staff and service population.
  • Performs other duties as assigned.
