Case Manager
About The Position
The Idaho Youth Ranch is seeking a Case Manager to join their team at the new Residential Center for Healing and Resilience (RCHR), a 64-bed psychiatric residential treatment facility (PRTF) for youth. This role is crucial in supporting the mission of nurturing hope, healing, and resilience in Idaho's youth by providing essential case management functions and duties. The Case Manager will collaborate with the clinical and medical team, client families, and referral sources to meet the comprehensive needs of the clients. This is a unique opportunity to be part of establishing a new standard for residential care.
Requirements
- Social Work Licensure or Bachelor’s degree in behavioral science or related field with 1 year experience with children in a social service setting or 5 years experience with children in a social service setting
- Ability to complete annual certification of CANS
- Demonstrate knowledge of Medicaid’s Youth Empowerment Services Program
- Demonstrate a working understanding of HIPAA and Medicaid guidelines and ability to maintain youth confidentiality and privacy
Responsibilities
- Provide case management for youth receiving inpatient residential care
- Assess, plan, link, coordinate, and monitor options and services that address a youth and family’s needs
- Help clients gain and maintain access to services and providers
- Provide skill-building services specific to goals identified in the individualized treatment plan to the point where the client may be safely, efficiently and effectively treated in the least restrictive service level
