About The Position

Cayuga Counseling Services, Inc. is seeking a Part-Time Case Manager in Cayuga County to provide Care Management under the Medicaid 1115 wavier program. This is an in-office position, and the hours are Monday - Friday 10am - 2pm.

Requirements

  • Detailed oriented, highly organized
  • Able to communicate expectations clearly
  • Strong computer skills, including experience with Microsoft and electronic documentation.
  • Knowledge of social services and community resources
  • Strong documentation and organizational skills.
  • The ability to work independently.

Responsibilities

  • Conduct outreach to referrals per program guidelines.
  • Navigate eligible members to programs within the Network.
  • Review lease and housing documentation, ensure documentation meets all requirements and conducts outreach to follow up on paperwork needs in order to assist eligible members.
  • Meets with clients to gather documentation and review program requirements.
  • Addresses member's care plan goals.
  • Completes surveys with clients who have been served through the wavier program.
  • Maintains strict Compliance with HIPPA and all federal, state, and agency confidentiality regulations, policies, and procedures.
  • Document all services and outreach per program policies and procedures.
  • Attend all required trainings.
