About The Position

Southern Kentucky Rehabilitation Hospital, a joint venture with Select Medical & Vibra Hospital, is seeking a Case Manager (RN or MSW). This role is responsible for coordinating health care decisions, ensuring treatment plans improve quality and outcomes, managing care across the continuum, promoting cost-effective care, assuring hospital-based services meet utilization management criteria, and implementing safe discharge plans. The Case Manager also assesses psychosocial needs and provides interventions as part of the discharge planning process. Primary job functions include Clinical Interventions/Discharge Planning, Care Planning Management, Fiscal Management, and Payer/Referral Management.

Requirements

• Current Licensure per state guidelines in a clinical or related discipline OR a Bachelor's or Master's in health or human services discipline.
• Previous experience in Case Management and Discharge Planning preferred.

Nice To Haves

• CCM Certification Preferred.

Responsibilities

• Coordination of health care decisions using a systematic approach to assure treatment plans that improve quality and outcomes.
• Coordination of care across the continuum.
• Promotion of cost-effective care within the allotted time frame.
• Assuring payments of hospital-based services meeting patient-related utilization management criteria.
• Implementation of safe and appropriate discharge plans.
• Assessing the psychosocial needs of the patient and providing intervention as part of the discharge planning process.
