Provide case management service to clients as outlined in the case management procedure manual in accordance with program guidelines. This role involves coordinating services for individuals who are being diverted from commitment to the Department of Children and Families, attending court hearings, and participating in treatment team meetings. The Case Manager will develop and maintain client assessments and service plans, ensuring all documentation is completed accurately and in accordance with agency standards. This position also requires monitoring client progress, participating in case conferences, and completing all required training.
