About The Position

This position is responsible for providing individualized and highly flexible support to people who have a serious mental illness utilizing a bio-psychosocial rehabilitative approach to service. The Case Manager will be required to develop, implement and evaluate individualized plans of care with clients on an ongoing basis.

Requirements

  • Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.W
  • Registered Nurse or Registered Practical Nurse with current registration with the C.N.O.
  • Bachelor’s Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W
  • Bachelor’s Degree with an applicable college diploma in Addictions and current registration with the C.A.C.C.F /or Grandfathered Post-Secondary Degree
  • Work with other professionals using strong problem solving and communication skills
  • Demonstrated ability to build partnerships with community partners
  • A valid driver’s license
  • Use of a vehicle, which will be used in conducting agency business

Nice To Haves

  • Canadian Certification in Psychiatric/Mental Health Nursing
  • Proficiency in both official languages

Responsibilities

  • Providing client needs identification through ongoing assessment and client directed care planning
  • Implementing client centered plan of care that focuses on strengths, wellness and empowerment to support recovery and goal achievement
  • Providing crisis prevention and intervention planning with the hope of preventing a relapse and/or hospitalization
  • Acting as a resource person or an advocate to assist individuals in obtaining information/access to services/supports which are deemed needed
  • Liaising with family/significant other and natural supports to facilitate the clients’ links within the community
