About The Position

The case manager is responsible for using sound professional judgment and best practices to ensure quality case management services are provided to the service population, especially in crisis situations, while maintaining compliance with established state and federal standards. This role involves monitoring an assigned caseload of clients participating in a short-term life management program, establishing program plans/goals, and evaluating client progress through routine meetings. The Case Manager acts as an advocate for clients to acquire services that will enable them to functionally cope with their environment. Key responsibilities include intake, coordinated assessment, developing individualized housing plans, making appropriate community referrals, maintaining client contact, and ensuring timely completion of housing plans. The position also requires participation in workshops for professional growth and maintaining confidentiality.

Requirements

  • Bachelor’s in Social Work, Psychology, Criminal Justice, Sociology or related field of study.
  • 1+ year related experience and/or training including working in Crisis intervention.
  • Proficiency in Spanish/English (written and spoken language skills), required.
  • Must maintain regular and acceptable attendance.
  • Driver’s License with a clear record.
  • Must have daily use of a vehicle without prior notice.
  • Must be available and willing to travel to various locations and with such frequency as the business need dictates.
  • Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others.

Responsibilities

  • Coordinate referrals, service planning and documentation of services for assigned caseload.
  • Interview applicants to determine eligibility for program enrollment based on established guidelines and requirements.
  • Conduct needs assessments, obtain pertinent information, and establish program goals for eligible clients.
  • Refer applicants to other agencies if not appropriate for the program.
  • Develop client intake and case plans.
  • Instruct clients in completing necessary paperwork, including goals and/or budget forms.
  • Complete orientation check-off forms and obtain release of information forms when appropriate.
  • Complete social histories on new clients each week.
  • Meet regularly with clients to discuss and evaluate their progress, preparing accurate and up-to-date records.
  • Monitor client behavior, group attendance, medication, and movement.
  • Receive incident reports and enforce disciplinary actions for infractions of house guidelines.
  • Plan, coordinate, and/or facilitate life management classes to assist clients in obtaining skills to functionally cope with their environment.
  • Obtain speakers and workshop presenters as needed.
  • Serve as a mentor and provide positive examples of proper hygiene, personal appearance, records retention, budget planning, scheduling, etc.
  • Refer clients to appropriate programs or other agencies based on clients’ needs and in accordance with their program plan.
  • Complete discharge plans for clients exiting the program.
  • Transfer file contents to non-active files.
  • Meet all deadlines required by program supervisor and partners.
  • Meet all federal and state regulatory guidelines and standards applicable to this position.
  • Maintain a caseload in accordance with agency policies and procedures and contract standards.
  • Participate in workshops, seminars, education programs, and other activities that promote professional growth and development.
  • Maintain a minimum number of weekly contacts with clients.
  • Engage and involve clients in the casework process.
  • Continually assess ongoing changes in behavior, circumstances, or conditions that may affect client safety.
  • Compile, prepare, submit, and maintain accurate records, files, forms, statistics, and additional information in accordance with agency policies, licensing, and/or contract requirements.
  • Participate in weekly case staffing with case managers.
  • Maintain confidentiality in all areas of the service population and program operations.
  • Demonstrate exceptional customer service by placing the child, family, Veteran, or client first to support the mission to 'Empower people to build better lives for themselves, their families, and their communities.'
