About The Position

The mission of SW Resources (subsidiary of Goodwill Industries of the Summit) is to provide vocational services and employment to individuals who have disabilities, enabling them to achieve their full potential. We offer a wide variety of services for businesses, allowing them to run efficiently and effectively while helping to create jobs and opportunities to help individuals live independently and productively. The case manager will bear primary responsibility for the general case management of an assigned caseload of clients at SW Resources in Parkersburg, WV.

Requirements

  • Bachelor’s Degree with one year of related experience, or equivalent combination of education and experience required.
  • Ability to obtain CPR and First Aid Certification within 30 days of hire.
  • Ability to pass criminal background check.
  • Valid driver's license required.
  • Must be 21 years of age or older.

Nice To Haves

  • Familiarity with the Division of Rehabilitation (DRS) organization, including DRS billing practices, preferred.

Responsibilities

  • Completing assessment duties as assigned.
  • Meeting, orienting, and instructing individuals to the assessment process.
  • Preparing assessment reports, as well as daily documentation and case notes.
  • Ensuring individual service plans are completed during the first 30 days, then annually.
  • Monitoring progress and behaviors of client employees on caseload.
  • Providing linkage and referral for other services as appropriate or prescribed.
  • Serving as an advocate for client employees.
  • Preparing and presenting new referrals during Admission and Placement meetings.
  • Coordinating placement or job retention activities to include preparing and assisting individuals with job search, utilization of community resources, marketing, and consulting with employers.
  • Participating in Quality Assurance of client employee records.
  • Assisting supervisors in phone calls regarding work.
  • Assessing Division of Rehabilitation Services (DRS) referrals in all phases of training.
  • Assisting clients with payroll onboarding through web-based HRIS system and answering any questions that may arise.
  • Attending DRS meetings as needed.
  • Serving as a job coach when needed.
  • Providing transportation for clients as needed.
  • Filing client records as needed.
  • Other duties as assigned.
