About The Position

Southwest Behavioral and Health Services is seeking a dedicated and empathetic Case Manager to join our dynamic Kingman team. The Case Manager serves as a central support within the interdisciplinary team, working primarily with SMI adults. This role involves conducting assessments, assisting members with community resources, and providing coordination of care both inside and outside the agency. We are looking for someone who enjoys coordinating with people, has problem-solving abilities, is organized, and possesses excellent communication skills.

Requirements

  • Minimum of 21-year of age
  • Bachelor’s degree in the behavioral health field plus one year of relevant experience in behavioral health
  • A combination of experience and education totaling 2 years is acceptable.
  • Valid AZ driver’s license & motor vehicle clearance.
  • Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive within eight (8) weeks of hire.

Responsibilities

  • Completing intake paperwork and referrals
  • Completing annual behavioral health assessments, including service planning and support planning
  • Facilitating CFTs and ARTs
  • Facilitating groups
  • Identifying gaps in care for our members
  • Completing SMI and SED evaluation
