Provide casework services and advocate for clients in the Holocaust Survivor Program. This role involves conducting intakes, providing information and referrals, screening and assisting clients with applications and re-certifications for entitlements and benefits. The Case Manager will assess clients in their home, hospital, or other institutional settings, advocate on their behalf, and arrange for services. They will develop, implement, and follow up on care plans, provide supportive counseling, and maintain ongoing communication with clients, family members, and other collaterals. The position requires maintaining complete and accurate case notes using the program's data system, preparing required documentation within organizational time frames, and reporting on statistics according to Selfhelp standards. Active participation in supervision and staff meetings is expected, along with timely reporting of emergent issues to the supervisor. The role also involves facilitating and assisting at social, recreational, and group activities, and potentially supervising social work students. Adherence to organizational standards includes completing annual mandatory trainings such as Sexual Harassment Prevention, IT Security Awareness, Compliance, OSHA, and HIPAA. The position supports the organization's mission by striving for excellence and maintaining positive interpersonal relationships with co-workers, and adhering to policies regarding absenteeism and appearance.
