About The Position

The Case Manager is responsible for developing and overseeing student plans, acting as a liaison between students, parents, and agencies, and ensuring compliance with program policies and procedures. This role involves direct interaction with students, parents, and staff, as well as comprehensive documentation and reporting. The Case Manager also plays a crucial role in maintaining a safe and supportive environment, modeling positive behavior, and contributing to the overall quality improvement of the program.

Requirements

  • Must adhere to the facility and daily schedules.
  • Must develop and oversee the completion of the MAP Plan and Graduation Plan.
  • Must serve as the primary program liaison.
  • Must provide monthly progress notes and weekly student updates.
  • Must facilitate court hearings, Probation Officer visits, student visits, home passes, and progress review meetings.
  • Must attend and assist with the facilitation of daily Positive Skill Development Group meetings and treatment groups.
  • Must manage and update assigned student files.
  • Must complete Serious Incident Reports.
  • Must complete required reports and documentation in a timely manner.
  • Must comply with and implement Rite of Passage Policies and Procedures.
  • Must ensure the highest standards are maintained to prevent illegal, unethical, or improper conduct and to ensure program compliance.
  • Must assist with the correction of deficiencies and quality improvement efforts.
  • Must attend and participate in all required site and student meetings.
  • Must attend all required training and staff development classes.
  • Must ensure the safety, health, and welfare of staff and students at all times.
  • Must provide encouragement, guidance, and resources to staff and students.
  • Must model and ensure all program norms are upheld without compromise.
  • Must act as a positive role model and mentor for both staff and students.
  • Must treat others with respect, confront negative behavior, and support confrontation.
  • Must oversee on-site family visits.
  • Must assist other staff members with emergency situations.
  • Must participate in Site, Region and/or Company community events as required.
  • Must complete other duties as assigned, verbally or in written form.

Responsibilities

  • Provides monthly progress notes and weekly student updates.
  • Facilitates court hearings, Probation Officer visits, student visits, home passes, and progress review meetings.
  • Attends and assists with the facilitation of daily Positive Skill Development Group meetings and treatment groups.
  • Manages and updates assigned student files in conjunction with administrative assistance.
  • Completes Serious Incident Reports and notifies appropriate individuals/agencies.
  • Completes required reports and documentation in a timely manner, providing management with reports and advising of problematic situations.
  • Complies with and implements Rite of Passage Policies and Procedures.
  • Ensures the highest standards are maintained to prevent illegal, unethical, or improper conduct and to ensure program compliance.
  • Assists with the correction of deficiencies and quality improvement efforts.
  • Attends and participates in all required site and student meetings.
  • Attends all required training and staff development classes to ensure annual training hours are completed.
  • Ensures the safety, health, and welfare of staff and students at all times.
