Targeted Case Management (TCM) is designed to ensure that individuals gain access to needed medical, social, educational services as well as other agencies whose functions are to provide the support, training and assistance required for a stable, safe and healthy community life. TCM services are provided to individuals who are experiencing severe and persistent mental illness. As an integrated health organization, Acenda values the use of evidence-based practice or EBP's. The Integrated Case Management Service (ICMS) is an outreach program serving individuals 18 years and older who reside in Gloucester and Cape May Counties. Services will be available to all eligible consumers with the intent to eliminate barriers to service regardless of location (in catchment areas) and cultural or linguistic orientation. The ICMS program includes targeted case management services. ICMS assist consumers to reach and maintain their optimal level of functioning in the community.

Requirements

  • Bachelor's Degree in Human Services or Psychology.
  • Professionalism and a commitment to excellence in care
  • Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record

Nice To Haves

  • At least 2 years of experience in mental services with adults

Responsibilities

  • Engages consumers at the point of hospitalization and participates in the discharge planning process to assist with the transition to the community
  • Provides post-discharge follow-up to support the consumer with community integration
  • Conducts assessments within correctional facilities and attends court hearings to advocate for persons served.
  • Provides supportive case management services in the community
  • Works as part of a TCM team in providing services to persons in the community
  • Completion of all required documentation as per program standards
  • Maintain required levels of service per program standards
