In this full-time role, you'll provide individualized service management for participants receiving Day Support, Respite, Summer Camp, and Employment Services. Requirements

  • Bachelor's degree in a related health or human services field, preferred; however, a combination of education, training, and experience may be considered instead of a degree, with a minimum requirement of a high school diploma or equivalent.
  • Relevant specialized training, certifications, and work experience.
  • At least two years of experience working with individuals with disabilities or medical concerns.
  • Demonstrated experience in service management and related documentation.
  • Strong knowledge of community resources, human services systems, and applicable regulations.
  • Excellent communication, analytical, and documentation skills.
  • Ability to work independently.
  • Ability to manage multiple priorities.
  • Ability to travel between program locations using personal or alternative transportation.

Responsibilities

  • Overall coordination and management of services for assigned participants across multiple programs.
  • Reviewing intake documentation, gathering additional information as needed, and developing individualized service plans (ISPs) in collaboration with individuals, legally authorized representatives, caregivers, support coordinators, and other stakeholders.
  • Completing annual assessments.
  • Maintaining accurate and comprehensive documentation.
  • Monitoring progress toward ISP goals through documentation review and on-site observation.
  • Completing required quarterly reports.
  • Serving as the primary point of contact and advocate for participants and their support networks.
  • Attending and facilitating planning and problem-solving meetings.
  • Interfacing with community partners and agencies.
  • Maintaining current knowledge of community resources.
