Case Manager
About The Position
A medical facility located near Winchester, Massachusetts is seeking full-time, permanent Case Managers to join their team! This is a full-time, direct hire position with a pay rate of $45.00-$53.00/hourly. The role involves collaborating with an interdisciplinary team to develop and guide treatment plans that align with patients’ needs, goals, and preferences. Responsibilities include coordinating with team members to establish discharge plans and appropriate contingency strategies, participating in the planning and implementation of the patient discharge process to ensure a smooth transition of care, facilitating weekly team conferences, coordinating treatment plan updates and modifications, and completing case management addendums along with all required documentation in a timely manner. The facility boasts a great leadership and administration team.
Requirements
- Must have active license as a RN, Social Worker, Occupational Therapist, Physical Therapist, Speech Therapist, or in Rehabilitation Counseling
- CCM or ACM certification required OR must be obtained within two years
Responsibilities
