One need not be Catholic to join our workforce nor to access our services. This program serves youth and their current caregivers due to the incarceration of the youth's parent(s). It is a prevention and supplemental behavioral health program that collaborates with a multitude of community resources with the aim of providing stability and improving functioning of family members. Services also provide the caregivers opportunities to enhance their parenting strategies.

Responsibilities

  • Under direction, ensure that the activities of this position and relevant programs are consistent with the mission, vision, and values of Catholic Charities, Diocese of Cleveland.
  • Work with children, caregivers, and parents to address unmet needs and support them as they navigate through their barriers.
  • Provide direct service either in the office, via telehealth or in the field/homes.
  • Initiate and coordinate interventions, review and monitor progress, and reassess needs to ensure successful outcomes.
  • Assist clients in developing formal and informal community supports, while partnering with local agencies and resources.
  • Provide advocacy on behalf of clients.
  • Prepare and complete all required case recordings in an electronic medical record, administrative reports, statistical reports and related paperwork or database records in a timely and professional manner.
  • Attend clinical and administrative staff meetings, and multi-discipline team meetings as required.
  • Review treatment plans and progress of cases with supervisor for continued support, guidance and approval.
  • Perform other duties as necessary to accomplish objectives.
