About The Position

Detroit Recovery Project, Inc. (DRP) is a Certified Community Behavioral Health Clinic (CCBHC) that provides integrated behavioral health and recovery support services for individuals experiencing mental health and substance use challenges. Through a whole-person approach to care, we help individuals build sustainable recovery and improved quality of life. Our recovery ecosystem focuses on five key areas: Health & Wellness Spirituality Employment Education & Training Family & Recovery Support. Detroit Recovery Project is seeking a Case Manager to support prevention, outreach, and participant engagement services within our community-based programs. This position plays an important role in helping individuals access resources, develop individualized service plans, and connect to supportive services that promote long-term health and wellness. The Case Manager will work closely with the Prevention Project Coordinator and community partners to identify participant needs, facilitate referrals, provide outreach and prevention services, and support individuals in achieving their personal goals. Requirements

  • Bachelor's degree in Human Services, Social Work, Psychology, Public Health, or a related field required.
  • Minimum of one (1) year of professional experience providing case management, outreach, prevention, or community-based services required.
  • Certified HIV Tester in the State of Michigan required or ability to obtain certification within an approved timeframe.
  • Strong communication, customer service, organizational, and documentation skills.
  • Ability to build positive relationships with participants, community partners, and colleagues.
  • Proficiency in Microsoft Office Suite.
  • Valid Michigan Driver's License and reliable transportation required.

Nice To Haves

  • Experience working with individuals affected by substance use disorders, mental health challenges, or other vulnerable populations preferred.
  • Qualified Mental Health Professional (QMHP) designation preferred.
  • Training in Motivational Interviewing preferred.

Responsibilities

  • Develop and maintain individualized service plans with measurable goals and objectives.
  • Assist participants in identifying needs, accessing resources, and overcoming barriers to care.
  • Conduct community outreach activities to recruit and engage participants in prevention programming.
  • Provide prevention education, referrals, linkage to care, and warm handoffs to community resources and treatment providers.
  • Conduct HIV and HCV testing in accordance with program and regulatory requirements.
  • Maintain a minimum caseload of ten (10) participants receiving case management services.
  • Coordinate services with community organizations, healthcare providers, and referral partners.
  • Administer required pre- and post-program evaluation tools and participant assessments.
  • Maintain accurate and timely participant documentation, case notes, and data entry within agency systems.
  • Monitor participant progress and provide ongoing support to encourage successful outcomes.
  • Participate in staff meetings, trainings, community events, and quality improvement initiatives.
  • Support the mission, vision, and values of Detroit Recovery Project through professional and ethical service delivery.
  • Perform other duties as assigned.
