We are searching for Master’s-prepared Clinical Case Manager for our Social Services Inpatient Team. Social Services Clinical Case Managers at Fairfax are part of an interdisciplinary treatment team charged with providing quality clinical case management services to our patients from admission to discharge. Clinical Case Managers are empowered to assess their patients’ needs, to collaborate with patients to create safe and appropriate discharge plans, to participate in diagnostics and treatment planning, and to work with our patients’ family/support/outpatient team for coordination of the discharge plan. Requirements

  • A bachelors or master’s degree from an accredited college or university in a mental/behavioral health field is required.
  • The desire to help our patients improve their lives.
  • Obtain an Agency Affiliated Counselor credential unless you have a clinical license.
  • Successfully pass the Handle With Care course completed during New Employee Orientation.
  • A positive attitude and outlook.
  • An understanding of the importance of regulatory-driven deadlines and the ability to stay organized while needing to pivot based on competing important priorities.

  • An MSW is preferred.

Responsibilities

  • Conducting Psychosocial Assessments to document patient history and to identify preliminary barriers to discharge.
  • Performing diagnostic discharge planning/case management starting from the day of admission.
  • Collaborating with outpatient providers and supports to ensure a successful transition for patients.
  • Identifying and recommending family and/or community resources such as housing, treatment providers, and other recognized healthcare disparities.
  • Participating in interdisciplinary treatment team meetings as an integral professional, clinical member.
  • Acting as liaison for hospitalized patients with various community agencies and resources.
  • Clinical documentation in the medical record.
  • Facilitating therapeutically beneficial meetings between patients and outside supports to coordinate discharge planning.
