The Case Manager- NORC position involves providing comprehensive social services to older adult clients and their families. This includes conducting in-home and in-office assessments using standardized instruments (i.e. PHQ-9, NSI), developing and implementing individualized written case plans, and offering individual and family counseling and guidance in problem resolution. The role requires assisting clients with screening eligibility, applying for, and advocating in securing benefits and entitlements, as well as arranging for direct provision of services such as transportation, homecare, meals on wheels, public assistance, Medicare, Medicaid, emergency cash relief, legal aid, protective services, vocational placement, medical and psychiatric examination and therapy, and housing. Case Managers also evaluate clients' capacity to manage their own affairs, assist in securing financial management or guardianship arrangements, and disburse JASA relief funds or client financial management services. They establish cooperative relationships with other community agencies, conduct ongoing reassessment of client needs, monitor service efficacy, and revise case plans as appropriate. Additionally, the role involves informing supervisors of crisis situations, conducting group counseling, community education, and caregiver support groups, representing the agency at community conferences and meetings, participating in training and unit meetings, and completing required case records, reports, and statistics, utilizing computers.
