Under the supervision of the Specialized Health and Social Services (SHSS) program supervisor, this role provides complex and comprehensive public health social work and case management services for children with special health care needs and their families, as well as children and high-risk pregnant women seeking services from local, state, and federal programs. Responsibilities include conducting Personal Care Services and Community First Choice assessments and reassessments, providing information and referral services, conducting presentations, and communicating program services to consumers. The role also involves providing training, consultation, and technical assistance to program contractors and providers, attending team and professional development training, and working as a team member. The position requires working under minimum supervision with considerable latitude for initiative and independent judgment, completing administrative duties and monthly reports, and attending work on a regular and predictable schedule. The employee actively participates in disaster response and/or recovery or Continuity of Operations (COOP) activation.
