About The Position
Performs complex case management work. Work involves reviewing and tracking case management activities; and assisting in developing program goals, objectives, and procedures. May assign and/or supervise the work of others. Works under limited supervision with considerable latitude for the use of initiative and independent judgment.
Requirements
- Experience in case management or social services work
- Graduation from an accredited four-year college or university with major course work in social work, behavioral sciences, social sciences, public health or related field is preferred
- Valid Texas Driver's License Required
- Knowledge of local community resources for social service, mental health and healthcare
- Skill in developing, implementing and monitoring service and treatment plans
- Knowledge of current principles and practices of planning and program implementation
- Knowledge of organization and administration of public health and/or human service programs
- Knowledge of program monitoring and policy analysis
- Skill in communicating effectively orally and in writing
- Skill in interviewing
- Skill in organizing and developing programs independently
- Skill in establishing effective and constructive working relationships
- Skill in use of personal computers and related software applications, including e-mail
- Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community
Responsibilities
- Maintains a working knowledge of community resources
- Coordinates referrals to community agencies and tracks outcomes of referrals
- Completes client assessments and care plans by identifying psychosocial, economic and physical needs of clients
- Documents all client encounters
- Completes and submits monthly reports
- Maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the client
- Reviews encounters and records of community prevention workers
- Identifies problem areas, service gaps, and areas that are barriers to services in the Lubbock community
- Coordinates organizational and community groups that are working to address social determinants of health
- Develop and implement educational programs
- Assists with the development of policy and procedures
- May supervise or coordinate the work of community workers
- Plan goals and objectives for assigned programs
- Assist in budget preparation
- Assist with health department social media
- Additional duties as assigned
