About The Position

Performs complex case management work. Work involves reviewing and tracking case management activities; and assisting in developing program goals, objectives, and procedures. May assign and/or supervise the work of others. Works under limited supervision with considerable latitude for the use of initiative and independent judgment.

Requirements

  • Experience in case management or social services work
  • Graduation from an accredited four-year college or university with major course work in social work, behavioral sciences, social sciences, public health or related field is preferred
  • Valid Texas Driver's License Required
  • Knowledge of local community resources for social service, mental health and healthcare
  • Skill in developing, implementing and monitoring service and treatment plans
  • Knowledge of current principles and practices of planning and program implementation
  • Knowledge of organization and administration of public health and/or human service programs
  • Knowledge of program monitoring and policy analysis
  • Skill in communicating effectively orally and in writing
  • Skill in interviewing
  • Skill in organizing and developing programs independently
  • Skill in establishing effective and constructive working relationships
  • Skill in use of personal computers and related software applications, including e-mail
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community

Responsibilities

  • Maintains a working knowledge of community resources
  • Coordinates referrals to community agencies and tracks outcomes of referrals
  • Completes client assessments and care plans by identifying psychosocial, economic and physical needs of clients
  • Documents all client encounters
  • Completes and submits monthly reports
  • Maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the client
  • Reviews encounters and records of community prevention workers
  • Identifies problem areas, service gaps, and areas that are barriers to services in the Lubbock community
  • Coordinates organizational and community groups that are working to address social determinants of health
  • Develop and implement educational programs
  • Assists with the development of policy and procedures
  • May supervise or coordinate the work of community workers
  • Plan goals and objectives for assigned programs
  • Assist in budget preparation
  • Assist with health department social media
  • Additional duties as assigned
