About The Position

The Senior Friendship Centers has an opening for a dynamic Case Manager – Lee County. The Case Manager is accountable for assessing, planning, implementing, monitoring, and evaluating actions required to provide case management and outreach services to eligible seniors. This position meets with seniors in their homes to complete assessments in person. Other related work can be completed remotely from home. Candidates must be comfortable driving to multiple locations using their personal vehicle (mileage reimbursement included). Must be willing to drive daily throughout Lee County. The successful candidate will be someone with the ability to communicate with patience and compassion to seniors who may be in a crisis situation, has intermediate computer skills, and exhibits our core values of: Respect, Compassion, Integrity and Collaboration. Requires a bachelor’s degree in Social Work, Sociology, Psychology, Gerontology, Human Services, or a related field, or any equivalent combination of training and experience which provides the requisite knowledge, ability, and skills to perform the essential functions of the job.
Intermediate computer skills are required; including Microsoft 365 and the ability to quickly learn, the organization’s computer software applications for records, reporting, and billing.
Satisfactory clearance of a fingerprint-based, level II background screening, and pre-employment substance abuse testing is required.
Must be authorized to work in the United States.

Nice To Haves

Three years of human service experience counseling clients preferred.
Bilingual preferred (English/Spanish)

Responsibilities

Assess the needs of older adults who have been referred to Senior Friendship Centers and screened as possible recipients of Title III, Community Care for the Elderly, Alzheimer's Disease Initiative, Home Care for the Elderly, and other programs.
Perform an in-home assessment with clients to evaluate the impact of services.
Assess, plan, implement, and monitor service delivery plans with clients utilizing community resources.
Perform required follow-up evaluations of services.
Complete the Department of Elder Affairs (DOEA) required annual assessment of each client.
Meet productivity goals as outlined by the Supportive Aging Services Director.
Complete client reports, logs, and case file documentation per program requirements.
Remain current on DOEA grant program guidelines, including attendance at training programs.
Participate in fundraising and outreach activities to promote the organization’s mission.
Report any incident of abuse or neglect as mandated by state guidelines.
Must comply with HIPAA regulations and OSHA laws and safety guidelines.
Attend staff meetings, training, and seminars as required.
Must comply with all Senior Friendship Centers personnel policies.
Must protect the safety and health of staff, volunteers, participants, and others by observing and following the organization’s safety and incident policies.
