About The Position

This is an exciting opportunity to make a real difference in the lives of our patients as a Case Manager at Perimeter Behavioral of Missouri - Saint Robert. In this role, the Case Manager will be coordinating care and communication for adolescents with behavioral challenges and medically complex physical conditions.

Requirements

Bachelor’s degree in social work, psychology, nursing, or related field.
At least twenty-one (21) years of age

Nice To Haves

Valid Missouri Driver's License is preferred
Experience in a mental health setting (preferably with adolescents), and knowledge of child/adolescent growth and development, and the ability to relate to adolescents with substance abuse or mental health problems is preferred.

Responsibilities

Serve as the main contact from adolescents and their families, offering support and guidance.
Assess the needs of adolescents with behavioral and medical conditions and develop individualized care plans in collaboration with the healthcare team.
Monitor patient progress and outcomes, adjusting care plans as needed to meet evolving needs and goals.
Advocate for the rights and needs of patients and families, assisting them in navigating the healthcare system and accessing necessary resources.
Provide education and support to families regarding treatment options, coping strategies, and community resources for ongoing support.
Maintain accurate and confidential case documentation, including care plans, progress notes, and communication logs, in accordance with hospital policies and regulatory requirements.
