The Colorado Coalition for the Homeless is dedicated to preventing homelessness and creating lasting solutions for individuals, families, and children experiencing or at risk of homelessness across Colorado. They advocate for and provide a continuum of housing and services to enhance the health, well-being, and stability of those they serve. Their philosophy centers on the belief that everyone has a right to adequate housing and healthcare, and they work to remove barriers to these rights. As a Case Manager I, you will collaborate with case managers, supervisors, integrated health teams, and community partners to foster long-term solutions for individuals experiencing or who have formerly experienced homelessness. Your responsibilities will include developing and implementing care plans, coordinating services, monitoring client progress, providing advocacy and support, facilitating referrals, and maintaining case documentation. Services are delivered using principles such as motivational interviewing, stages of change, strengths-based case management, housing first, harm reduction, cultural responsiveness, and trauma-informed care. This role will focus on outreach, facilitating referrals to mainstream services and housing, and crisis intervention.
