La Casa Norte’s mission is to serve youth and families confronting homelessness. We provide access to stable housing and deliver comprehensive services that act as a catalyst to transform lives and communities. Our programs, including Casa Corazón (Drop-In and Emergency Beds) and Solid Ground Supportive Housing, aim to transform the lives of homeless and at-risk youth (ages 16–24) by providing them with the stability, support, and guidance they need to thrive. The Case Manager is responsible for providing comprehensive case management, advocacy, and supportive services to homeless and at-risk youth. This position supports youth in developing life skills, securing stable housing, and accessing educational, vocational, and healthcare resources. Work schedules vary depending on program assignment, typically Tuesday - Saturday with evening and weekend flexibility required. Positions are located across multiple neighborhoods, including Humboldt Park, Back of the Yards, and Logan Square.
