About The Position

The Department Case Manager supports individuals receiving OPWDD and other Regulatory Agencies to ensure the foundation necessary to achieve their highest level of independence as they transition into adulthood. Through person-centered planning, service coordination, and ongoing case management, the Case Manager facilitates access to social, vocational, and community-based opportunities in collaboration with the agency and external providers. The role includes intakes, enrollment for, afterschool and weekend respite services, day habilitation, camp, employment services, and community engagement. The Case Manager is responsible for the development, implementation, and ongoing monitoring of individualized service and habilitation plans in accordance with OPWDD/and other Regulatory Agencies. This includes ensuring services promote individualization, independence, inclusion, and productivity, as well as coordinating supports among internal agency departments, families, and external service providers to ensure continuity and quality of care. The Case Manager will work with the Department Senior Director to ensure all enrollment, reporting and compliance documentation is completed in a timely manner for all current and future programs.

Requirements

  • Must have a valid NY State Driver’s License
  • Experience within a human service-related environment.
  • 2 years’ experience working with the Special Needs Population
  • Knowledge of OPWDD Policies

  • Preferred BA/BS degree in Special Education, Social Work, Psychology- or any other Human Service-related field

Responsibilities

  • Oversees client care by assessing treatment needs during the intake and enrollment process; developing, monitoring, and evaluating individualized treatment plans; facilitating interdisciplinary collaboration; and monitoring staff performance.
  • Manages the admission of new clients by reviewing records and applications and conducting comprehensive orientations.
  • Determines client needs through intake interviews; coordinating therapeutic, medical, psychosocial, and psychiatric evaluations; and reviewing clinician evaluations, treatment objectives, and care plans.
  • Establishes and coordinates treatment programs by developing schedules and routines; overseeing service delivery; and arranging necessary resources, including transportation and escorts.
  • Monitors ongoing cases by tracking attendance; evaluating treatment effectiveness and client responses; advocating for services and entitlements; securing additional resources; and offering ongoing client support.
  • Maintains accurate and compliant client records by reviewing case documentation and systematically recording significant events and progress updates.
  • Communicates client progress through weekly interdisciplinary meetings and formal evaluations; disseminates outcomes, barriers, and contributing treatment factors to clinical teams and families.
  • Facilitates client transition by reviewing and strengthening transition plans; coordinating post-transition services; educating and training family members; and providing appropriate community resources.
  • Improves treatment outcomes by analyzing, evaluating, and redesigning programmatic operational processes; implementing evidence-based improvements; and updating policies and procedures accordingly.
  • Prepares comprehensive reports by collecting, analyzing, and summarizing treatment outcomes, performance metrics, and trends; compiling statistical data; and completing grant and subsidy applications.
  • Maintains professional expertise by engaging in continuing education, reviewing professional literature, networking with peers, and participating in relevant professional organizations.
  • Strengthens departmental and organizational reputation by proactively assuming responsibility for new initiatives and identifying opportunities to add value and improve service delivery.
