The Department Case Manager supports individuals receiving OPWDD and other Regulatory Agencies to ensure the foundation necessary to achieve their highest level of independence as they transition into adulthood. Through person-centered planning, service coordination, and ongoing case management, the Case Manager facilitates access to social, vocational, and community-based opportunities in collaboration with the agency and external providers. The role includes intakes, enrollment for, afterschool and weekend respite services, day habilitation, camp, employment services, and community engagement. The Case Manager is responsible for the development, implementation, and ongoing monitoring of individualized service and habilitation plans in accordance with OPWDD/and other Regulatory Agencies. This includes ensuring services promote individualization, independence, inclusion, and productivity, as well as coordinating supports among internal agency departments, families, and external service providers to ensure continuity and quality of care. The Case Manager will work with the Department Senior Director to ensure all enrollment, reporting and compliance documentation is completed in a timely manner for all current and future programs.
