The job of case manager is done for the purpose of providing individualized support, advocacy, and guidance to opportunity youth (ages 16–24) facing economic, academic, social, and employment barriers. The goal is to support students in achieving high school equivalency, developing life and career skills, accessing wraparound services, and transitioning successfully into employment, training, or postsecondary education. The case manager develops, monitors, and maintains case management plans, delivers group and individual support, and fosters community partnerships to ensure barrier reduction and progress toward personal and program goals. This position will serve students/participants in the Open Doors Program and in the YouthBuild Program.
