About The Position

Care Lync is looking to hire a full-time Case Manager to provide the activities necessary to meet the needs of the clients assigned to the consolidated care team. This social services position earns a competitive base salary. There are also opportunities for bonuses! We provide excellent benefits and a family environment. We are a social services agency focused on providing support coordination and case management services to individuals across Nevada that have physical disabilities and are enrolled in Medicaid waiver programs. Requirements

  • BS/BA degree in related course work
  • No prior experience required
  • Experience with Microsoft Office, state-maintained databases.
  • Must maintain proficiency in company sponsored training and certifications.
  • Maintain updated state related Clearances.
  • Successfully complete and maintain training courses as required or amended by program regulations.
  • Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration, copies must be maintained on file with Care Lync.

Responsibilities

  • Provide the activities necessary to meet the needs of the clients assigned to the consolidated care team.
  • Discern the disabilities and accompanied needs of our participants as well as helping them achieve their goals.
  • Carefully make a plan for each participant and modify it annually or when there is a significant medical or social change.
  • Communicate with other individuals and entities to ensure seamless coordination between physical, behavioral, and support services.
  • Develop and maintain professional working relationships with participants and their families.
