About The Position

Willow Tree Residence is a 130-unit congregate care supportive housing program located in East Midtown Manhattan. The facility is unique in that it is co-located with a women’s shelter that occupies the first 6 floors of a 21-story building and offers a street facing Article 28 clinic on the first floor dedicated to providing primary and behavioral health care services. Willow Tree provides 79 permanent, supportive housing units for chronically homeless single adults with a serious mental illness [SMI], a substance use disorder [SUD], or those who may have a co-occurring SMI and SUD. Candidates are referred through the Department of Homeless services [DHS]; however, the facility is funded by DOHMH. The remaining 51 units are “fair market” and reserved for low-income tenants, referred from the local community.

Requirements

  • Bachelor’s degree from an accredited college or university.
  • An Associate’s degree with three (2) years of experience working with homeless, mentally ill and chemically addicted individuals or other special needs populations.
  • Ability to work cooperatively with peers and other staff to serve the needs of the clients as effectively and efficiently as possible.
  • Excellent oral, writing, and listening skills are a must.
  • The ability to work well within a highly pressurized, deadline environment, while meeting the short-term and long-term mandates of the program.
  • An ability to interface with clients as well as all levels of staff.
  • Knowledge of case management software as well as proficiency in Microsoft Office Suite.

Responsibilities

  • Conducts required assessments, general case management and direct care services to clients.
  • Supports clients in addressing identified needs, building the skills and stability necessary to regulate their lives, achieve greater independence, and transition to permanent housing or other appropriate community-based housing options.
  • Provides referrals and links clients to appropriate internal and external resources.
  • Assists with other cases as needed, and ensures all required protocols are followed.
  • Maintains client charts in accordance with program standards and ensures documentation remains complete, accurate, and audit-ready at all times.
  • Performs other related duties as assigned.
  • Provides case management services and assists clients in the creation of and adherence to their Service Plans by identifying short and long-term objectives that can be achieved.
  • Conducts intake and assessments, makes necessary referrals for services, and completes/updates psycho-social assessments of tenants.
  • Provides direct services, linkage to services and coordination of services for Willow Tree residents.
  • Counsel residents on money management, medication management, daily living activities and housing maintenance.
  • Accompany and advocate on behalf of the residents with community-based services, as needed.
  • Create and lead groups addressing functional and psychosocial needs of residents.
  • Participate in weekly clinical team meetings and receive individual supervision.
  • Inputs all client information and updates in a timely fashion into the AWARDS system as well as the client’s physical chart.
  • Maintains accurate and up-to-date written client documentation as required and in accordance with City/State oversight agency guidelines.
  • Maintains liaison with representatives from community organizations.
  • Performs other duties as assigned by the Team Leader, Director of Social Services, and/or Program Director.
