About The Position

Hamilton Families is dedicated to ending family homelessness in the San Francisco Bay Area. Since 1985, we have provided essential services to unhoused families, focusing on prevention, shelter, transitional housing, children's well-being, and facilitating a return to stable housing. The Transitional Housing Program (THP) serves 20 families for 12-18 months, offering comprehensive social services to address the root causes of homelessness and promote independent living. Services include case management, referrals, life skills training, health services, children's services, and housing assistance. This Part-Time Temporary Case Manager role will provide case management and support to a limited number of families to ensure continuity of care while new staff are onboarded, focusing on their stability and transition to permanent housing.

Requirements

  • A minimum of 2 years of experience providing support services to families and/or individuals in a human services setting, OR a bachelor’s degree in social work, psychology, or related field from an accredited college or university and a minimum of one year of experience providing support services to families and/or individuals in a human services setting.
  • High School Diploma or GED required.
  • Experience working with and sensitive to the needs of homeless families.
  • Able and willing to work with diverse staff and participants.
  • Experience in a crisis setting and able to respond appropriately to emergencies, including contacting appropriate staff and/or contacting and interacting with police, fire, and medical personnel as needed.
  • Experience providing support services to participants with mental health related issues, substance abuse, domestic violence, HIV/AIDS related issues, Child Protective Service agencies, HIPAA and crisis intervention techniques.
  • Excellent written and verbal communication skills.
  • Proficient in Microsoft Office (Word, PowerPoint, Outlook, Excel, etc.).
  • Able to make regular entries into a web-based participant database.
  • Must be available for occasional evening hours as work schedule includes some evenings.
  • Essential functions of the position may require use of a telephone, working at a standard computer terminal, and the ability to follow reasonable ergonomic recommendations, sit for long periods, stand, reach, bend, lift, and carry up to 40 pounds.
  • CPR and First Aid certification required within first 6 months of hire.
  • Criminal background check and fingerprint imaging required post offer.
  • TB (Tuberculosis) clearance and documentation required post-offer.

Nice To Haves

  • Experience working in Case Management, preferred.
  • Experience working in a residential facility, preferred.

Responsibilities

  • Provide case management services to a select number of residents with a focus on exit planning and family success.
  • Develop, implement, and document family action exit plans that address participants' needs, including housing options, recovery issues, children's needs, and short-term goals.
  • Identify, network with, and refer participants to appropriate community-based services.
  • Communicate and collaborate with case managers, supervisors, program staff, and community partners to provide optimal support to families.
  • Follow and reinforce all program policies, rules, and expectations for families.
  • Complete and maintain required handwritten and computer-based records, files, correspondence, and statistics in a timely, clear, thorough, accurate, and legible manner.
  • Compile, complete, and submit required reports and information documenting services provided and program outcomes.
  • Review and present the progress of each family in regular case conferences.
  • Prepare reports and presentations as required.
