Hamilton Families' mission is to end family homelessness in the San Francisco Bay Area. Since 1985, we have been meeting the most urgent needs of unhoused families in our community. Our programs specialize in preventing family homelessness when possible, providing shelter and transitional housing to families in times of need, supporting the wellbeing and academic achievement of children who experience homelessness, and quickly returning families to safe and stable housing throughout the Greater Bay Area. The Transitional Housing Program located North of the Panhandle serves 20 families for 12-18 months. Families participating in our residential programs receive comprehensive social services to identify and address factors contributing to their homelessness to enable them to return to stable, independent housing and economic self-sufficiency, including: case management with referrals and linkages to community services and benefits, employment and life skills training, access to basic health services, children’s services, and innovative and direct assistance in securing permanent housing. The PT temporary Case Manager provides a limited number of families with case management and support to assist them with gaining stability and obtaining permanent housing, while we onboard new staff. The purpose of this role is to ensure continuity of care with a select number of families.
