Requirements

  • Requires bachelor’s degree Human Services or related field
  • Communication, organization, time management and clinical skills
  • Ability to work independently in a community-based setting
  • Valid Colorado Driver’s license with a clean driving record

Nice To Haves

  • Bilingual Spanish preferred, but not required

Responsibilities

  • Provides prevention/early intervention, case management, psychoeducation, and other clinical interventions to consumers at Stages
  • Supports and closely coordinate mental health care with the consumer’s clinical care team and the primary care provider
  • Works as an integral member of the Stages treatment team to facilitate Utilization Management for insurance authorizations, including organizing re-auth schedule, compiling relevant notes and submitting to insurance, and coordinating with Medicaid funding sources
  • Attend collaborative treatment team meetings and reviews of consumers
  • Provides crisis de-escalation when needed
  • Assist eligible consumers in gaining access to needed medical, social, educational, and other services
  • Provide an assessment of needs and service plan development
  • Empowers consumers to reach their goals and provides role modeling as well as technical expertise to accomplish goals
  • Provide transportation for consumers in agency vehicles when needed
  • Will work closely with outside organizations on resources and care coordination
  • Using an evidence-based model of measurement-based care tracks success for each individual consumer
