About The Position

The New Life Recovery Center is seeking a diverse, energetic, self-starting case manager to join its team. The case manager provides active case management to individuals with mental illness, substance use, or co-occurring disorders who need these services. The role coordinates services and resources that support clients’ basic needs, including psychiatric or MAT appointments, medical appointments, and court hearings. It also supports follow-through and continuity of care through discharge, referral, placement, follow-up appointments, and liaison services. The case manager attends weekly treatment team meetings and maintains contact with referral sources, such as Probation and DSS, as needed. The starting salary range is $18.98–$21.83 per hour.

Requirements

• High school diploma.
• At least one year of experience in a setting working with individuals with substance use disorders and/or possible co-occurring mental illness.

Nice To Haves

• Bachelor’s degree in education, social work, psychology, or a related human services field.
• Registration as a QMHP or trainee.
• CSAC qualification.
• Speaking multiple languages; additional compensation will be considered for new employees who do so.
