About The Position

Community Partners in Action (CPA) is seeking a Case Manager for their Work Release Program in Hartford, CT. CPA, founded nearly 150 years ago, is dedicated to criminal justice reform and supporting individuals in and out of prison through various programs including reentry and housing, youth initiatives, and alternatives to incarceration. The organization serves over 4,000 individuals annually throughout Connecticut and is guided by a strong commitment to Belonging, Equity, Diversity, and Inclusion (BEDI). The Work Release Program (WRP) provides residents with safe housing, basic needs assistance, employment readiness and placement, and life/work skills development to promote self-reliance, responsibility, and accountability.

Requirements

  • Bachelor’s degree, preferably in Social Sciences or two-year degree in substance abuse counseling (experience may be substituted year-to-year).
  • Experience in case management and client supervision.
  • Knowledge of the criminal justice system.
  • Ability to interact with people of diverse backgrounds.
  • Strong interpersonal and communication skills.
  • Computer literate.
  • Bi-lingual (English/Spanish) skills required.
  • Valid driver’s license and reliable transportation.
  • Some evening and holiday hours may be required.

Responsibilities

  • Commit to mission and program goals.
  • Promote a culture of respect, empathy, mutual support, belonging and work to eliminate any unconscious bias or discrimination in our beliefs, rules and systems.
  • Supervise residential clients in compliance with Department of Correction security requirements.
  • Assess all new intakes to determine programming needs.
  • Develop service plans to include employment, life and financial skills, release prevention, release planning.
  • Provide orientation to the program for all new intakes.
  • Provide and document individual counseling for assigned caseload.
  • Facilitate and document groups using modality specific to clients.
  • Provide effective crises intervention.
  • Maintain client confidentiality.
  • Conduct substance screening specimen testing and testing protocol of program.
  • Maintain contact and coordination with employers and outside agencies monitoring client programming.
  • Work in collaboration with the Employment Specialist to ensure that all client needs are addressed.
  • Work with area landlords to develop a pool of housing opportunities for clients.
  • Serve as a role model to residents and staff by modeling pro-social behaviors.
  • Solicit resident satisfaction feedback.
  • Establish and maintain appropriate boundaries with residents and members of their identified support group.
  • Actively participate in the overall effort to meet performance outcomes set by CT DOC and CPA.
  • Participate in related seminars, panels and workshops to enhance visibility of program.
  • Seek and engage in annual training to increase overall professional skills.
  • Perform other duties related to effective advocacy for employment of ex-residents, as necessary.
  • Maintain a flexible work schedule.
