About The Position

The Case Manager will be responsible for ensuring case management to clients in a family shelter setting in an effective and efficient manner. This position is located in Staten Island, NY. Work Schedule: Monday - Friday, 9:00 a.m. - 5:00 p.m.

Requirements

  • Bachelor's Degree required
  • Previous experience with the homeless population needed.
  • The ability to establish workload priorities and balance a diverse caseload.
  • Must have exceptional communication skills both orally and in writing with individuals and groups regarding complex and sensitive issues or regulations.

Responsibilities

  • Completes family intake, comprehensive family history, and maintains accurate and timely family case records.
  • Responsible for designing social service plans in cooperation with new residents.
  • Set and follow-up on long and short-term client goals and objectives.
  • Secures entitlements for clients and serves as an advocate for families in conflict.
  • Responsible for advocacy and providing assistance to clients in obtaining permanent housing.
  • Make referrals to special housing agencies.
  • Meet with clients to develop strategies to meet their housing needs.
  • Develops an individual program of housing readiness, parenting skills, prevocational and vocational counseling as necessary.
  • Works collaboratively with family monitors.
  • Participate in and attend the Interdisciplinary Team and Administrative meetings weekly.
  • Submit all monthly reporting in a timely manner.
