About The Position

Community Hope is looking for a full-time Case Manager for our Brining Veterans Home (BVH) program. This hybrid position involves providing case management and supportive services to veterans. Services include intake, assessment, housing identification and stabilization, rent and move-in assistance, budgeting, financial entitlements/benefits, case conferences, referrals for adjunctive services, family meetings, advocacy, and liaison with various agencies, landlords, and real estate professionals. The role also includes linkage and referral for mental health, substance abuse, legal, and other service needs. Responsibilities also encompass eligibility screening, maintaining accurate records, meeting with participants in various community settings, entering data into an electronic health record, transporting veterans as needed, attending and organizing outreach events, and documenting service delivery while ensuring veteran confidentiality. The Case Manager will also function as a team member by covering shifts, communicating information, and assisting co-workers.

Requirements

  • Bachelor’s Degree in Social Work, Psychology, or a related field.
  • 1 year of case management experience, preferably in working with veterans or the homeless.
  • Must possess a valid driver’s license, with a good driving record.

Responsibilities

  • Providing case management services and eligibility screening to veterans in the Bringing Veterans Home (BVH) Program.
  • Providing case management and supportive services including intake, assessment, housing identification and stabilization, rent and move-in assistance (financial), budgeting, financial entitlements/benefits, case conferences, referrals for adjunctive services, family meetings, advocacy, and liaison with other community and governmental agencies, landlords, real estate agents, and real estate brokers.
  • Providing linkage and referral for mental health, substance abuse, legal, and other service needs.
  • Providing eligibility screening to applicants and collecting and maintaining accurate and detailed records.
  • Meeting with participants in community settings such as motels, shelters, meal sites, libraries, transit hubs, jails, hospitals, vehicles, private residences and other outdoor or unsafe locations.
  • Entering intake, contact, housing stability plans, temporary financial assistance, and other information in the electronic health record in a timely and accurate manner.
  • Transporting veterans, as needed in agency vehicles.
  • Attending and organizing outreach events in the community including outreach in encampments, under bridges, outdoors, in shelters, stand downs, soup kitchens, houses of worship, or other places where the homeless congregate.
  • Documenting service delivery accurately and timely while ensuring veteran confidentiality.
  • Functioning as a team member by covering shifts, communicating all relevant information to other team members, and covering co-workers’ duties as needed.
