About The Position

The Case Manager oversees the provision of services and supports to individuals in accordance with each individual's identified outcomes as well as The Arc's Mission Statement and Guiding Values. The Case Manager is accountable to the person receiving support, their team, and the agency.

Requirements

  • High School Diploma or GED required.
  • Ability to communicate openly and effectively, verbally and in writing
  • Ability to establish and maintain effective positive working relationships with individuals, families, and agency personnel
  • Exhibits the ability to exercise patience, understanding, and concern for each individual's health, growth, safety, and enjoyment of life
  • Demonstrates skill in supporting individuals through crises as well as circumstances of day-to-day life
  • Ability to maintain confidentiality, demonstrate discretion and integrity
  • Background checks as required by licensing agencies
  • TB Test
  • Valid driver's license
  • Proof of auto insurance
  • Ability to move about
  • Must be physically able to evacuate individuals in the event of an emergency
  • Ability to see within normal parameters
  • Ability to hear within normal range
  • Adaptability to a fast-paced environment
  • Computer (Intermediate Level)

Nice To Haves

  • Associate degree in Human Services preferred.
  • Two years previous social services work experience with case management responsibilities strongly preferred
  • One year in the field of developmental disabilities preferred

Responsibilities

  • Coordinates with agency licensing personnel for initial and annual licensing of Foster Care and Assisted Living Homes
  • Assures placements are appropriate by agency standards and procedures, and licensing regulations
  • Assures ongoing compliance with licensing regulations and HUD requirements
  • Refers and assists individuals/families to appropriate community services/entitlements
  • Develops and writes each person's Habilitation Plan
  • Performs monthly checks of each residence and assigned vehicle to ensure they are in good condition and maintenance schedules as followed
  • May provide direct care support, as directed by supervisor
  • Makes reports in cases of known or suspected abuse and neglect under Alaska Statutes and The Arc of Anchorage Abuse/Neglect Policies
  • Directs the activities of assigned staff or functions
