Requirements

  • Bachelor’s degree in social work or a related field of study
  • Candidate must have a valid driver’s license which has been held for at least 1 year; in the last 3 years, candidate must have obtained no more than 3 license violation points
  • Ability to engage and build rapport with populations with high support needs.
  • Strong oral and written communication skills
  • Detail-oriented and resourceful with strong time management and organizational skills
  • Proficiency in Microsoft Office suite and other standard business technology
  • Valid driver’s license is required for this position.

Nice To Haves

  • One-year of case management or related work experience preferred.
  • Bilingual/multilingual preferred, but not required.

Responsibilities

  • Independently manage a caseload
  • Conduct comprehensive intake assessments.
  • Conduct regular field visits to engage and build rapport with chronically homeless individuals.
  • Work with clients to develop goals and service plans.
  • Provide referrals and direct assistance to support clients applying and/or attending appointments for mental health services, medical care, substance treatment, vital documents, benefits, legal/immigration services, and permanent housing.
  • Document all interactions with or on behalf of clients, monitor and document client progress toward service plan goals.
  • Maintain familiarity with social services and resources available both on-site and in the community.
  • Coordinate client care with other service providers; attend regular case conferences with clients and other service providers, as required.
  • Attend regular supervision sessions with direct supervisor.
  • Drive agency vehicle to and from client locations and appointments
  • Perform additional duties as assigned.
