As a Case Manager, you will work under the clinical supervision of a licensed mental health professional to deliver case management services for youth in care. You will coordinate treatment activities, facilitate communication between stakeholders, and help youth and families prepare for successful reintegration into their communities. This role is part of the Gallatin Primary Assessment Center, a secure short-term facility serving boys ages 13 to 17 who require assessment and stabilization. The center offers continuous observation, clinical evaluation, mental health care, and life skills programming during initial placement. Youth typically remain for about 30 days, receiving individual and group therapy, educational services, victim-impact awareness, and family engagement. Small group living units support informed placement decisions and safe transition to longer-term residential or community settings.
