About The Position

The Harm Reduction Services Case Manager provides low-threshold, participant-centered case management to individuals enrolled in the Harm Reduction Services Program. The Case Manager engages people who use drugs and individuals at risk for overdose, HIV, Hepatitis C, and other health and social harms. Services include intake and assessment, development and monitoring of individualized service plans, supportive counseling, group facilitation, referrals, and care coordination. The Case Manager supports participants in accessing medical care, substance use treatment, benefits, housing, and other essential services while promoting safer substance use practices and overall health and stability.

Requirements

  • High School Diploma or GED required.
  • 1–2 years providing case management, outreach, or direct services to high‑need populations.
  • Knowledge of harm reduction principles.
  • Ability to discuss safer injection and overdose prevention.
  • Ability to respond to an opioid overdose using naloxone.
  • Strong engagement and counseling skills.
  • Proficiency in basic computer applications including Microsoft Word, Outlook, and data entry systems, with the ability to complete timely and accurate documentation.

Nice To Haves

  • Bachelor’s Degree preferred.
  • Bilingual Spanish preferred.

Responsibilities

  • Conduct outreach internally and in the community to engage and enroll eligible participants into Harm Reduction Services.
  • Complete program intakes, obtain informed consent, and collect required documentation.
  • Assess participant needs and develop individualized service plans.
  • Provide individual supportive counseling and facilitate harm reduction or psychoeducational groups.
  • Deliver overdose prevention education and safer substance use education.
  • Provide referrals and coordinate care with medical and social service providers.
  • Follow up on referrals and document outcomes.
  • Participate in interdisciplinary case conferences and team meetings.
  • Maintain accurate and timely participant records.
  • Monitor participant progress and update service plans.
  • Advocate on behalf of participants to reduce barriers to care.
  • Support program data collection and reporting requirements.
  • Assist with overdose response efforts and naloxone distribution.
  • Perform other duties as assigned.
