About The Position

The Case Manager will ensure the smooth, cooperative functioning of the treatment program by serving as a member of the Program Treatment Team. The goal is to integrate clinical, milieu, and direct-care efforts and issues. This role fosters a Family-Centered approach in working with the family members of a resident and a youth. The Case Manager will obtain and compile information from various sources and maintain a comprehensive record on each resident. They will serve as an advocate for the resident during placement, ensuring health, safety, and residents' rights. Additionally, the Case Manager will coordinate care by serving as a liaison between members of the multidisciplinary treatment team, including therapists, nursing staff, psychiatrist, consulting providers, recreation staff, program staff, management, referral source, family, and the resident.

Requirements

Bachelor’s Degree in Human Services OR Social Work field OR 5 years of experience in Human Services
Up to date with all required trainings in current position.
A minimum of one year of childcare experience required.
Valid state issued driver's license.
Must be at least 21 years of age.
Legally authorize to work in the United States and provide verification.
Able to pass an Enhance Background Check through the Idaho Dept. of Health & Welfare.
Within two months of hire date, completion of in-service training per agency policy
During the evaluation period ensure that all relevant data has been gathered including psychological evaluations, psychiatric reports, school data, medical information, prior treatment records, and other relevant records.

Responsibilities

Serve as a member of the Program Treatment Team with the collaborative goal of integrating clinical, milieu, and direct-care efforts and issues.
Foster a Family-Centered approach in working with the family members of a resident.
Foster a Family-Centered approach in working with the family members of a youth.
Obtain and compile information from various sources and maintain a comprehensive record on each resident.
Serve as an advocate for the resident during the resident's placement including the assurance of health, safety and residents' rights.
Coordinate care by serving as a liaison between members of the multidisciplinary treatment team, including therapists, nursing staff, psychiatrist, consulting providers, recreation staff, program staff, management, referral source, family and the resident.
Schedule and conduct multidisciplinary meetings including those for the master treatment plan, treatment plan reviews, discharge meetings, and other case conferences as necessary.
Submit written reports of these meetings within established time frames.
Maintain a current, comprehensive case record of each resident, working cooperatively with the therapist, program staff, other agency departments, and clinical records department.
Update team members and the clinical records office of changes in case status including guardianship, caseworkers/parole officers, addresses and phone numbers, approved contacts, and discharge plans.
Serve as a resource to multidisciplinary team members in relation to accurate and effective evaluation, planning, implementation and accountability for each resident's program.
Work closely with the therapist and referral agency representative to maintain the level of communication necessary for effective, comprehensive treatment.
Provide direct service to residents, including life space counseling, advocacy, discipline, instruction, and information giving.
Offer residents a nurturing and supportive relationship while communicating a clear set of values and expectations.
Maintain an understanding of agency and program treatment philosophy, policies and procedures, and conduct case management within these parameters.
Maintain consistent communication with each resident's parents/guardians and referring agency regarding treatment program, progress, incidents, and significant developments occurring during the resident's placement.
Respond to questions and concerns as they arise.
Conduct personal and professional phone calls daily with residents to ensure they are staying in contact with supportive persons outside the agency.
Coordinate resident visits with outside agencies, family, and professional personnel.
Provide supervision as needed and coordinate supervision/transportation with the Program Manager.
Ensure completion of all intake and discharge documentation including the completion of resident inventories within the required time frame.
Coordinate discharge planning and aftercare for residents.
Work efficiently and effectively as a member of a professional team.
Complete all other duties as assigned by Supervisor.
