The Case Manager provides client care consistent with Caring Connection policies, procedures, and code of conduct. This role is responsible for compiling and communicating monthly chart reviews or requirements per facility, updating Adult Day Services (ADS) care plans quarterly, and communicating with caregivers, families, Care Managers, and physicians regarding a client’s condition. The Case Manager assists with eligibility determination of new admissions, admission assessments, and the development of person-centered care plans. They also assist with wellness screenings and assessments, provide support and resources to clients and caregivers, and participate with the Adult Day Services team in providing a safe and healthy environment. Additionally, the Case Manager assists with medical emergencies, trains ADS staff and new hires, makes best practice recommendations, completes grant compliance forms, and creates internal processes for accountability.
