About The Position

Overture Home Care is seeking a Case Manager for their Dallas location to support clients in the Collin County area. The agency provides RN-delegated home care and companion services to seniors and individuals needing assistance, aiming to help them live independently, safely, and with dignity. The Case Manager will enhance patient management quality, maximize satisfaction, and promote cost-effectiveness by overseeing the full patient care cycle, including assessing, planning, implementing, monitoring, and evaluating actions to meet clients' health and human services needs.

Requirements

  • Must have a High School Diploma / GED
  • Willingness to participate in on-call nights and weekends
  • Ability to pass an extensive background check
  • Professional communication skills

Nice To Haves

  • A certified case manager is a plus
  • Previous experience in scheduling, managing, and/or healthcare preferred
  • Previous experience with Axiscare not required but is a plus

Responsibilities

  • Scheduling and ensuring that the agency is adequately staffed to provide quality care to clients.
  • Coordinating the client’s visits and communicating with the Talent Acquisition team.
  • Completing on-site home admission assessments and supervisory visits for clients.
  • Developing the Care Plan for coordination of care for all clients.
  • Functioning as a liaison between the client, family, and the caregiver, actively eliciting feedback from the client.
  • Completing competency assessments and validations for caregivers.
  • Maintaining records to support and validate caregiver competency according to state rules and regulations.
  • Ensuring the provision of in-service/staff development programs that contribute to meeting agency objectives and standards and comply with all federal, state, and local regulations.
  • Participating in on-Boarding presentations as needed.
  • Ensuring the Core Values of Patience, Dignity, and Discretion are adhered to at all times.
  • Establishing and facilitating effective employee/client relationships.
  • Maintaining the confidentiality of all pertinent client care information to ensure client’s rights are protected.
  • Assisting in performing caregiver assignments when there are shortages (appropriate training needed).
  • Answering the staffing phone to adjust schedule due to unforeseen events.
  • Assisting with answering phones as needed.
  • Ad Hoc assignments as they are needed.
  • Being on-call as needed.
